Designated premises supervisor
If you are applying for a premises licence and will be selling or supplying alcohol you will need to have a named designated premises supervisor. They must hold a personal licence and be responsible for the day-to-day running of the premises.
As part of the premises licence application, a consent form for a designated premises supervisor will need to be returned to us. You can download it from this page and then submit with your premises licence application.

If you are currently listed as a designated premises supervisor on a premises licence and want to be removed you can let us know through the Gov.UK website.
